While you are trying to save people's lives in the game, you will still be cutting people's abdomens and internal organ open with a pizza cutter. You'll be using everyday items to perform surgery, like a lighter to burn wounds closed, a chainsaw to remove broken bones, and a vacuum to suck up blood. Blood is plentiful. And the patients you're operating on always seem to have been through horribly traumatic accidents, often with various objects poking out through various wounds.

There is some raunchy humor in the game (a joke about a patient with crabs, for example). And female patients are often in sexy lingerie. A dominatrix character drags a small man behind her by a chain attached to his nipples.

The inappropriate language here is mostly used in reference to the risqué jokes (see "Sex" above).

Many characters drink and smoke. One character has cigarettes stuck within his internal organs.
